A Systematic Review and Meta-analysis Highlights a Link Between Aerobic Fitness and Telomere Maintenance
VO₂max has long been considered the gold-standard indicator of cardiorespiratory fitness and endurance performance. But emerging research suggests it may also provide important insights into biological aging.
A recent systematic review and meta-analysis published in The Journals of Gerontology explored the relationship between aerobic fitness and telomere maintenance — a key marker associated with cellular aging. The study found that individuals with higher VO₂max values generally exhibited longer telomeres compared to less fit individuals, suggesting a link between cardiorespiratory fitness and healthier biological aging.
Importantly, the authors emphasize that achieving extremely high fitness levels may not be necessary. Individuals with VO₂max values in the 70th percentile or higher already showed significant benefits in telomere maintenance, reinforcing the value of regular endurance exercise and consistent physical activity.
The review also highlights the importance of objective VO₂max assessment through Cardio Pulmonary Exercise Testing (CPET) with respiratory gas analysis — the gold-standard methodology for evaluating the integrated response of the cardiovascular, respiratory, and muscular systems during exercise.
As interest in preventive medicine and healthy longevity continues to grow, CPET and VO₂max assessment are becoming increasingly relevant not only in sports performance, but also in clinical and wellness applications.
